The "petition for graduation" process sometimes seems mysterious. Below is a description of the process. A copiy of the generic petition form is below.  If you have questions about the Petitioning process, please contact Sharon Doty, doty_s@fortlewis.edu, 247-7397.

PLEASE REVIEW THE FOLLOWING INSTRUCTIONS:

1.       ID#:  Please use your student ID number and not your social security number.

2.       Name: Please print your name exactly as your wish it to appear on your Diploma.

3.       Graduation Date: Please be as accurate as possible and visit with your advisor to

determine when you anticipate graduating and then list the month and the year.  

i.e….April 2008, or Summer 2008 or, December 2008, or later. Please note:

students completing requirements during one of the summer sessions(s) must list

their graduation date as Summer and the year. You will however participate in the April Commencement as there are no graduation ceremonies in the summer.

4.       Advisor:  Please list the name of your advisor.

5.       Advisor Approval and Signature:  Your advisor must approve and sign your Petition

before you submit it to the Registrar’s Office.

6.       Degree:  This will be either a Bachelor of Arts or Bachelor of Science. Check your catalog

if you are uncertain.

7.       Principal Major:  This is your primary program in which you are seeking a degree.  

i.e. Psychology, Art, Business Administration, etc.

8.       Options/Concentrations if any:  This is the option that you have chosen to study within your Major.  i.e. Art K-12 Option, European History, Humanities, etc.  Not all Majors will have options.

9.       Second Major if any:  List another program in which you are seeking a degree. (optional)

10.   Intended Minors, if any:  A minor program that you have chosen to study in addition to your Major program. (also optional)

11.   Catalog Year:  Please list the year of the course catalog that you have been referencing to complete your Degree requirements.  Most students choose the catalog in effect the year they entered Fort Lewis College.  i.e.  2005-06, 2006-07, 2007-08.  

12.   Hometown:  This will be listed in the Commencement Program that is distributed during the

Commencement Ceremony.  

13.   Student Signature:  Your signature is required to make this a valid document before returning it to the Registrar’s Office.  

14.   Phone number, E-mail and mailing address:  Please provide us with the most current contact information, including a LOCAL mailing address to which we can send your Graduation checklist.
