Dear New Student:
On behalf of the Student Health Center, we welcome you to Fort Lewis College! The Student Health Center is conveniently located on campus, where we strive to take care of all your health care needs.
Please Be Aware:
Colorado State Law requires you to submit an immunization record with evidence of two MMR vaccinations (measles, mumps and rubella) to the Student Health Center. Colorado State Law also requires Fort Lewis College to provide information to all incoming freshman students living on campus about meningococcal (meningitis) disease.
A current copy of your immunization record must be submitted by: September 16 for the fall semester, January 27 for the spring semester, by May 8 for summer sessions One, Two, Three and June 12 for summer sessions Four & Five.
If you do not comply with this requirement, a $25 fee and a hold on making any registration changes will be placed on your student account.
Use the following information to assist you in obtaining and submitting records:
- Submit an official signed/stamped copy of your record. Have your doctor or school nurse complete the Release of Immunization Records & Certificate of Immunization forms found under the "Forms" tab or use the links above to access.
- We prefer that the immunization record be sent to the Student Health Center by scan/email to email@example.com. Other options include mailing to (Student Health Center, 1000 Rim Drive Durango, CO 81301) and faxing to (970-247-7621), by the above census date deadlines.
The Health Center will process your immunization record and respond with an email of receipt by the end of the business week.
The Student Health Center offers affordable and accessible health services to all students.
We are open Monday thru Friday from 8:30am - 12:00pm and 1:00pm - 4:30pm.
