Fort Lewis College Welcomes New Employees

Welcome! As a new member of the Fort Lewis College community, there are a few items you need to become familiar with, and a few items you are required to do prior to your first day on the job. To help guide you through your first few days as a Fort Lewis College employee, we've prepared a New Employee Checklist. Please review and print out this checklist to help ensure that you complete the required items.

Prior to your first day as an employee, please call the Office of Human Resources at 970-247-7428 to schedule an Employee Intake Meeting. At this meeting, we'll collect forms you've already completed, explain in detail your benefits and benefits options, help complete any necessary forms, and address any questions or concerns you might have about working at FLC.

Items needed to be completed prior to your first day as an employee are:

  1. Personal Data Form: This form gives us the biographical information necessary to enter you into the College's Human Resources and Payroll system. This information also allows you be be set up with Information Technology and the SkyCard office. If you are a faculty member, this information also starts the process to allow assigning your faculty load.
  2. I-9 Form: The Immigration Reform and Control Act requires that all newly employed individuals provide documents which establish identity and employment eligibility and to complete the I-9 Form. Section 1 of the I-9 Form must be completed no later than your first day of employment.

    Listed in the instructions of the I-9 Form are acceptable documents which must be presented during your Employee Intake Meeting. If you cannot bring the appropriate documentation with you to your scheduled Employee Intake Meeting, please notify Human Resources to reschedule your meeting and to postpone the start of your employment.

Fort Lewis College will employ only US Citizens and aliens authorized to work in the United States. Failure to comply with the provisions of this act will result in immediate termination of employment.

  1. Policy Access and Acknowledgement Form: After you review the following information and policies, please print and sign this form.

  2. W-4 Form: The W4 Form establishes your withholding amount for Federal and State income taxes.

  3. Direct Deposit Form: At Fort Lewis College, direct deposit of your payroll checks is a condition of employment. This form lets payroll contact your bank in order to directly deposit your payroll check into an established bank account.

  4. Social Security Statement Acknowledgement: Fort Lewis College does not pay into Social Security. By signing this statement acknowledgment, you are stating that you understand that Fort Lewis College does not pay into Social Security on your behalf.

  5. Retirement Plan Election Form: Since Fort Lewis College does not pay into Social Security, you are required to pay into an alternative retirement option. All Classified Staff must join Public Employees Retirement Association (PERA). Exempt Staff and Faculty must choose either TIAA-Cref or Valic (Unless they are currently active as a PERA employee or if they have previous service as a PERA employee). You may want to talk to an HR representative prior to making your final selection, but we've included this form here to encourage you to begin researching which option is best for you.
