Quickstart

A completed IRB application should include:

• Completed form - Exemption Application
• Copy of Informed Consent - Informed Consent form
• Copy of survey instruments (as applicable)

The package should preferably be transmitted as a single document.

To enure timely review, the document name should include:

• Type of application (full, expedited, exempt)
• Discipline related to research
• Last name(s) of principle investigator(s)

         For example:  Exempt_econ_miller_smith
