Music Forms for Students:

Student Recital - Use this form to submit your piece for inclusion in the Thursday student recital hour. Please fill in the form, save it to your M: drive, then attach it to an email to your applied teacher. Your applied teacher will forward approved forms for your inclusion in the recital. This form must be forwarded to your applied teacher no later than 5:00pm Monday the week of the recital.

Juries - Fill out the top portion of this form and make the appropriate number of copies for your student jury exam. Jury exams sign-ups will be posted at least one week prior to juries and will include the number of jury forms you need to bring.

Junior/Senior Recital Program - This is a form designed for creating your recital program and notes. Your program should be completed and sent to your applied teacher no later than 2 weeks before your recital date. Your applied teacher, and your Senior Seminar professor must have this amount of time to work with you on revisions. If you submit this information too late you run the risk of not having your programs printed for your recital, and of recieving a reduced grade.

Junior/Senior Recital Proposal Form - You must complete this form and turn it in two months prior to your recital date.
