Campbell Child & Family Center is a year-round early care and education program focusing on enhancing child development and learning through quality programming. Only families needing full-time care will receive placement at the center. Full-time is defined as at least 5 hours daily, Monday through Friday.
Families must pay $150 non-refundable security fee per child upon confirmed enrollment; paying the fee serves as confirmation of accepting your child's slot. Please refer to Tuition & Fees for information on rates and payment.
We request families ensure their children arrive at 9:00 a.m. and are in attendance through 12:30 p.m. every day to support routine and consistency at home and in school. During this time, children take part in highly interactive learning activities and become participants in the educational process.
Campbell Center places families from an extensive waitlist and has adopted several priority policies with regard to enrollment. Click Waitlist Information to review policies and instructions to get on our waitlist.

Enrolled families may access the Family Handbook of Policies & Procedures and print for your reference.
Physical Exams and Immunization Records
Due to state licensing regulations and policies, physical exams by a health care provider must be updated EVERY YEAR for children 2 ½ years and older (Tadpoles through Rainbow Fish classrooms), and EVERY 6 MONTHS for children up to 2 ½ years old (Caterpillar classroom). Use the Early Childhood Health Assessment Form below, however, a provider-signed copy of your child's physical exam from their office may replace the form.
Immunization records must be up-to-date upon your child's first day of childcare, OR notice of an appointment or written plan must be in your child's file within 14 days of first childcare day. Be aware that Colorado has implemented several steps in acquiring immunization records from parents: 1) Notice of Immunization Requirement, 2) Immunization In-Process Form, 3) Notice of Exclusion. Therefore, it is easiest for families to submit a copy of immunizations after every shot. Please visit CDPHE School Immunizations for more information of Colorado's policy on immunizations, exemptions, and child care.
All forms here are PDF format. After downloading, you may use the Fill & Sign feature to enter your information, then print. OR, open link and print forms, then enter information by hand. Be sure to sign and initial all forms where indicated; submit at the Front Desk. These forms are required AFTER you accept a slot offered to your child(ren) and pay $150 non-refundable security fee; you do NOT need to submit these forms while you are still on the waitlist.
