Accommodations for students with long-term disabilities

Follow these steps to apply to the Accessibility Resource Center and begin receiving accommodations for longterm disabilities. If you are seeking accommodations for temporary impairments, visit our temporary accommodations page.

IMPORTANT: It may take up to four weeks to review and process your request for accommodations. Please begin this process as early as possible prior to attending FLC or as soon as you become aware of your need. Accommodations requested after the start of the Semester are not retroactive to the first day of the Semester.

Returning students: If you have already registered with the Accessibility Resource Center, log in to your student portal to request accommodations at the start of each semester.

Step 1: Apply

You are responsible for identifying yourself to the Accessibility Resource Center as a student with a disability and requesting accommodations. To begin this process, please complete the online student application.

Step 2: Provide disability information and documentation

How will your disability likely impact you on campus? We want to understand the relevant impacts of your disability on various tasks, such as communication, reading, writing, housing, technology, dining, physical environment, and classroom/laboratory/online learning. Information on the impacts of physical (mobility, dexterity, endurance, etc.), perceptual, cognitive (attention, distractibility, communication, etc.), and behavioral abilities may be helpful as well.

Examples of helpful information and documentation include, but are not limited to:

  • Educational records, such as a high school accommodation plan (IEP, 504 plan, etc.)
  • Neuropsychological or educational evaluation
  • Statement from a health, mental health, or other service provider
  • Medical records
  • Audiology report
  • Vision assessment
  • Vocational assessment
  • Proof of accommodations used on standardized exams (SAT, ACT, etc.)

Submit documentation

If you are unable to submit documents through the secure link above, you may use one of the following methods:

  • Email: accessibilityresources@fortlewis.edu (Note: this email is not secure and is not HIPAA compliant)
  • Fax: 970.247.7070
  • Mailing address: 071 Reed Library, 1000 Rim Drive, Durango, CO 81301

Need help getting a diagnosis?

Any information and documentation you can readily share prior to step 3 is helpful and can prevent a delay in services. However, we recognize that barriers exist in presenting third party disability documentation. If you do not yet have the appropriate documentation, please proceed with Step 3.

Step 3: Intake appointment

This is our opportunity to ask you questions and learn how we can best support you. We will send you a request to schedule your intake appointment. While most of our appointments are conducted in-person, we do have virtual options available upon request.

Step 4: Growth!

Once you have registered with the Accessibility Resource Center and received accommodations, you will still need to request accommodations through the student portal at the start of each new semester. You will also be asked to check in periodically with a member of our team to make sure that your accommodations are the best options for you!
