Your appeal must be received by the deadlines listed below. Appeals will only be reviewed if they are complete, signed, and accompanied by appropriate documentation. Incomplete appeals and appeals received after the published deadline will not be reviewed. Appeal decision notifications will be sent to your FLC email address.
In making its decision, the Academic Standards Committee will review your appeal, the submitted you documentation, and your educational record. See the Student Privacy Rights (FERPA) page for information on what is included in your educational record.

Box A (Personal Statement): Please provide an explanation for your unsatisfactory academic progress, including any compelling and/or unforeseen factors or circumstances. If you can support any of the claims made in your personal statement with documentation, you are encouraged to do so (e.g. medical documentation, expense reports, obituaries, statements from a third party).
Box B (Plan of Action): Provide a detailed plan of action for to ensure your future academic success at Fort Lewis College, should you be allowed to continue as a degree-seeking undergraduate student.
