PURPOSE: In Fall 2016, Fort Lewis College implemented a new curriculum and a new set of academic policies that affected almost every student attending FLC. If you have been adversely impacted by these changes, you have the right to petition for accommodation using this form.
REVIEW PROCEDURE: The petition will be routed to the Registrar (or designee) for technical review and recommendation for action.
APPROVAL PROCEDURE: The petition, the technical review, and the Registrar's recommendation will be routed to the Provost’s designee for a decision. This decision is final. There is no appeal. Notification of the decision will be sent to the student’s FLC email account and to their primary advisor.
