Lockout - Tagout

The provisions of the Lockout/Tagout Program shall apply to all personnel at Fort Lewis College and the Hesperus Campus buildings and operations. As used in this program, the terms employee and personnel include student workers. 

General

This program applies to the servicing and maintenance of machines and equipment in which the unexpected energization or start up of the machines or equipment, or release of stored energy could cause injury to employees. This program establishes minimum performance requirements for the control of such hazardous energy.

Responsibilities

  • Each department director or dean is responsible for conducting a hazard assessment of his/her area of responsibility to determine whether the lockout/tagout program applies to the processes and equipment in his or her area.
  • Supervisory staff will insure all employees are instructed in the significance of the lockout and will comply with all aspects of this program.
  • Supervisory staff will ensure that each new or transferred affected employees whose work operations are or may be located where a lockout will occur shall be instructed in the purpose and use of the lockout or tagout procedure.
  • Employees are responsible to observe safety practices contained in the Lockout/Tagout program and to point out unsafe conditions to their supervisor.
