Vaccination requirements

To support the health of the campus community, students are required to be vaccinated against measles, mumps and rubella (MMR) and COVID-19 (beginning Fall 2021). We also strongly recommend that students get vaccinated against meningitis. You can read more about these requirements on our Immunization Requirements page and our COVID-19 Vaccine Requirement page.

Here's what you need to do

You must either provide a copy of your vaccination records OR have an exemption before the start of semester.

Provide proof of immunization

To demonstrate that you have fulfilled the MMR and COVID-19 vaccination requirements, all you need to do is upload a copy of your immunization records in your admission portal (log in as a returning user).

  • If you do not have your immunization records, you should be able to obtain them from your high school, a previous college, or your doctor.

If you have not yet been vaccinated against COVID-19, we encourage you to do so as soon as possible. If you are unable to get a vaccination before the start of the Fall 2022 semester, we will provide one for you the week prior to the start of the fall semester.

Request a vaccination requirement exemption

You have the right to request a medical or non-medical exemption from one or both vaccination requirements. Please see the FLC Student Health Center's Immunization Requirements page for more information about exemptions. You can also read our detailed COVID-19 vaccination requirement exemption policy on our Vaccine Policy page.

If you choose to request an exemption from the COVID-19 vaccination requirement, you can do so using either the Medical Exemption Request form or the Non-Medical Exemption Request form.

Please upload completed exemption request forms to your admission portal (log in as a returning user).
