Step 1: Fill out and submit EHS Driver Authorization form and complete Mandatory Annual Driver training, contact ehs@fortlewis.edu

Step 2: Request a rental from PPS motor pool: Transportation Services (Motor Pool) | Physical Plant | Fort Lewis

Step 3: Accident and Breakdown Reporting.

• A. For an accident contact EHS and PPS and fill out the accident report on EHS’s webpage or in vehicle glove box.
